Various vacuum blocks available left over from other projects all are brand new and never used

4 Nickel Plated Aluminum 5 port 1" X 1" X 2.5" with three 1/8" NPT threads and two 1/4" NPT ports all three small ports are on top and the 1/4" are one on each end $20 shipped

Not sure, it all depends on how many vacuum lines you have fed off of your intake manifold. To replace the four way one of the 5 ports would work.
