What causes a new crush washer to go bad within 2 months???

Maybe it wasn't annealed? Rinse it off with brakleen and hold a torch to it until it glows. Let it cool down and see if it works.

Bought a new brake hose kit... Comes with 2 washers... Gotta tell him not to overtorque it.

I would say not seated right or over torqued

Dirt or not tight enough, I've never seen a line leak from being too tight, If its too tight, usually you break the banjo bolt or strip the threads.
