The negative terminal clamp on my GF's Altima needed to be replaced. So I put on a new clamp but in order to have the extra slack I disconnected the cable to where it is attached to the body but it also looked like a ground , however after it is attached to the body it is grounded on the engine by the same cable. So my question is where its attached to the body really a ground? or just there to keep the cable tight in the engine bay. The car runs fine with the cable just running to the engine.

You should always have good grounds to the battery. Just make sure that the place where you ground it is decently free of paint for a good contact. It would probably be good to spray something protective over it like a zinc primer and then some clear coat so it won't rust. Wires can ground through paint,but it's better to have a metal to metal contact, in my opinion.

You need both grounds for everything to work properly. It may work and run ok now but you will have problems in the future. Not everything grounds to the engine so everything else grounds to the body and it runs through the wire you speak of.
Ya I went ahead and ran a cable from the old hole in the middle of the negative cable and attached it to the body so now it is grounded to the engine and the body.:
The biggest pain was running around to 2 autozones, and dealing with people that had no idea what I was describing to them.
